7-13-12 7-13-12 Chuck, look back on page 21 and before and after those pages. My contact information is there along with the required contract documentation needed to be part of this group. You can't go anywhere without your contract so you will have to locate or get another. After you have sent it, we will talk. I do not have a questionaire. That was someone else. Christine
